UV Map : A 2d texture map of a 3d object. The U (horizontal) and V (vertical) coordinates, are the axes (of the UV map). They control the location and orientation of the 2d textures applied to the 3D object.
Usually, UV maps are applied to models in a 3D modeling app, but are also created/changed in specialized 'UV mapping' apps.

Unwrap (of a UV map) - When the UV map co-ordinates are projected on to a flat plane as a 2d image, the UV map is said to have been "unwrapped" (in the sense of removing the peel/skin from an orange as a continous spiral and flattening it completely). These unwrapped UV map 2d images can be painted on in a paint program and made into textures to apply to a model, providing a method of making highly detailed (but flat) textures for models. Some 3D apps (like zBrush) allow painting on your model directly in 3D, however the results aren't always acceptable.
