Me rikey. What is the final format it should all be exported to? Are the sizes in it pixel perfect?

You did it wrong.
Do not add banner.png as sublayer! just add your image with png alpha channel w/o any background. During christmas multiple banners showed a blue top left corner cause ppl merged banner.png into their banner images.

(this image is already part of the default website layout and gets rendered under the banner image, so you must not include it in your banner)
