Well, first off:
[ 0] Stereo3D widget: shader0 error:Fragment shader failed to compile with the following errors:
ERROR: 0:8: 'texture2D' : no matching overloaded function found
ERROR: 0:8: 'rgb' : field selection requires structure, vector, or matrix on left hand side
ERROR: 0:8: 'assign' : cannot convert from '3X3 matrix of float' to '3-component vector of float'
ERROR: 0:9: 'texture2D' : no matching overloaded function found
ERROR: 0:9: 'rgb' : field selection requires structure, vector, or matrix on left hand side
ERROR: 0:9: 'assign' : cannot convert from '3X3 matrix of float' to '3-component vector of float'
ERROR: compilation errors. No code generated.
If that Widget did not gracefully deactivate itself afterwards, then you could have crash.

Were there any other obvious graphics problems (corruption, weird stuff happening visually, etc.) before the crash?

Perhaps, yes. Graphical Widgets can cause various Bad Things to happen, some slowly.

It's not likely, mind you, but he said that he had a "graphical glitch", so I assumed there was something going on that had a graphical symptom before the engine failed.
