You haven't read my posts then. I said it would require a 5 minute trip into the phpbb admin. The guidelines wanted are already in place as moderators already have to record details of moderation, such as when who and why.

The only difference with what I proposed is that these existing practices would be done in a read only public forum and moved into a private forum should anything private need to be discussed. No extra recording should take place (assuming all moderators are following the rules currently in place and doing as theyre supposed to).

No, we don't have "moderation rules". We have general rules that moderators are responsible to enforce, we have guidelines and constant discussions as to what are responsible actions in regard to each infraction, but there are no "rules" that moderators must follow. That doesn't mean that we don't get moderators in trouble for doing stuff they shouldn't do, it's just that the nature of the job requires some creativity, and alot of intelligence. Most of the stuff we can safely put into "rule" form we write bots like chanserv to do for us.
